  • Title: The Application Allogeneic Mesenchymal Stromal Cells In Combination with Autologous Fibrin Gel For Mine Blast Trauma During Wartime

Abstract

 Introduction. During wartime mine-blast trauma produces complex soft tissue destructins. Surgical operations and cell therapy strategies using mesenchymal stromal cells (MSCs) carried in fibrin have shown promising results in regenerative medicine.  MSCs are crucial for wounds healing due to their universal properties. Fibrin gel is a natural polymer involved in the coagulation process  that favors angiogenesis, extracellular matrix deposition and cell-interactions. Additionally, fibrin gel maintains paracrine functions of MSCs, providing tissue regeneration. The aim was to assess the therapeutic potential of autologous fibrin gel combined with allogeneic MSCs in the regeneration of wounds soft tissues after mine-blast trauma. Materials and methods. From 24  patients that were studied, 12  had previously been treated using traditional protocols, such use was with difficulties and complications in longlasting postoperative periods. Other group of 12 patients treated after beginning this study in addition to a regular surgical protocols of treatment were also receiving cell therapy with allogeneic MSCs (from 10 to 20 mln. cells/patient) combined with autologous fibrin gel  applied to the surface of a damaged tissue of the wound  for 3-12 days in repeated applications.  According to the approved rules, MSCs were isolated from umbilical cords of both sexes obtained with maternal informed consent after normal childbirth. MSCs were characterized according to the minimal criteria for defining MSCs (ISCT). Results. All patients recovered in bouth groups, but in second group process of wounds healing was much easier and faster. Regeneration process in wounds was faster, easier with short postoperative period and hospitalisation time of hospitalisation (3-5 days shorter in average). According to a microscope specimens evaluation major improvements in second group were noted with increased microvascular density (27.5 ves/cm2  versus 6.1 ves/cm2 in second group), increased collagen fibers density reduction of inflammatory mast cells in surgical sight  environment. Conclusions. Fibrin gel loaded with MSCs presented considerable potetial and better quality soft tissues regeneration of the wounds, increased microvascular density and faster healing process with significant improvement in clinical parameters.
